When does a tooth need a filling?

Minimal areas of tooth decay or broken tooth structure may be successfully restored using dental fillings. We only use composite (tooth colored) fillings at our practice. We take a very conservative approach when deciding if a tooth should have a filling or not. We don't want to drill on your tooth unless you really need it. A lot of times, if a tooth has a tiny cavity and it has stayed the same size for a long time, then we can just leave it alone and just keep an eye on it, instead of drilling on it and placing a filling.

This video goes into great detail about situations when a filling would benefit your tooth and situations when it is better to just leave your tooth alone.

Dr. Kokabi also answers a question we hear constantly: whether old silver fillings need replacing.

Do I really need a filling?

Dr. Kokabi works through when a filling is genuinely needed and when a tooth is better left alone. Chewing surface cavities are judged by sight and by whether the explorer catches in them, and the answer changes with the patient: a small dark spot on an 80 year old with no history of decay is treated differently to the same spot on a teenager who gets cavities often. Cavities between the teeth only show on x-rays, and very small ones are often watched rather than drilled. He also covers chipped teeth, abfraction lesions, old fillings and root canal treated teeth. Untreated decay usually does progress toward a root canal, but a small demineralized spot can stay stable if brushing, flossing and sugar habits improve.

Do old silver fillings need to be replaced?

Patients often come to us for a second opinion after being told all of their silver fillings have to go. An old amalgam filling that is smooth, uncracked and has no decay at its edges is usually fine left alone, even if it is decades old and worn. It does need replacing when it is cracked or broken, or when decay has formed where the filling meets the tooth, which shows as brown or black staining that catches an instrument. Replacing them because you dislike how they look is a separate conversation, and one we are happy to have.

Common questions

How much do fillings cost?

The price for a filling ranges from $300-500 based on how many surfaces of your tooth it covers. This is the price without insurance, if you have insurance then your out of pocket will probably be much less depending on how good your insurance is.

Do fillings hurt?

Usually no, because we get you numb so that you do not feel the procedure.

How long do fillings take to get done?

We usually block off one hour for 1-2 fillings. And about 90 minutes if you need 3 fillings the same day.
